Antimicrobial resistance (AMR) is a global health and development threat which causes death and disability, prolonged illness results in longer hospital stays, and the need for more expensive medicines and financial challenges for those impacted. WHO has declared AMR as one of the top 10 global public health threats facing humanity. 

Rising resistance is of particular concern for Gram-negative bacilli, such as Pseudomonas aeruginosa, Acinetobacter baumannii and Enterobacteriaceae (figure 1) and carbapenems are last resort antibiotics to treat the infections caused by these Gram-negative bacilli. Among the many drug-resistant bacteria facing the clinic, the cases of carbapenem-resistant Enterobacteriaceae（CRE）bacteria are rising shapely recently and need to be resolved in an urgent way.

The resistance mechanisms of these bacteria are diverse and adaptable, making the control and early identification of CRE infection difficult. Therefore, the subtyping of bacterial carbapenem hydrolases in clinical microbiology laboratories is very important to guide clinicians to use antimicrobial drugs rationally and effectively.

Resistance to carbapenems in these species is related either to combined mechanisms of resistance (overexpression of broadspectrum ß-lactamases together with efflux pumps, impermeability) or expression of carbapenem-hydrolyzing ß-lactamases, known as carbapenemases. In Enterobacteriaceae, carbapenemases represent the most important mechanism of resistance, making them potentially responsible for outbreaks.
